ESSC League Champs T-Shirt Order Form

Congratulations!!

Please note that all orders will be filled as closely as possible to your requested order. However, championship
t-shirts have been pre-ordered by the ESSC, and as such, size quantities are not completely flexible.

The allotted amount of T-Shirts per sport/team is as follows:
1. Slo-Pitch, Outdoor Soccer: 10-13
2. Ultimate, Flag Football: 9-12
3. Badminton: 2-3
4. Indoor Volleyball: 6-9
5. Floor Hockey, Indoor Soccer, Dodgeball, Basketball: 9-12

T-Shirt sizing was ordered based on the following percentages:
Small: 24% ......... Medium: 33% ......... Large: 33% ......... X-Large: 19% ......... XX-Large: 1%

If your team orders its shirts close to these percentages, you will be more likely to receive your requested quantities. You can order greater quantities and/or different sizes but there are no guarantees on receiving your exact order. The ESSC will distribute extra t-shirts once all teams have received their initial totals.
